Our dentist and team in Louisville, Kentucky, are in-network with Delta Dental® Premier and MetLife PPO, ensuring you receive the maximum benefits from your insurance. For patients with other insurance providers, we are happy to file out-of-network claims as a courtesy. We also accept various payment methods, including cash, checks, all major credit cards and CareCredit®. Our goal is to provide flexible financial options to meet your needs.
Now Offering an In-Office Membership Plan
Dr. Howell’s office is pleased to now offer an In-Office Membership Plan to patients currently not covered by a dental benefit plan. We want to help you prioritize your oral health, and this program allows for a complete evaluation of preventive care.
With this plan, you will receive:
- 3 cleanings per year
- 3 exams per year
- All X-rays
- 10% off all over services, excluding implant placement and restorations.
All for only $275 per person, $500 per couple and an additional $200 for each family member in the same household.
*This is not an insurance plan and is not subject to regulation by the state department of insurance.
*Plan membership cannot be combined with current dental insurance plans, no insurance claim will be filed for members under this plan.
*Total payment amount is due at the time services are provided. If Full payment is not received at the time of service, fee discount will be void.
* The plan is for individual use only and is non-transferable, has no cash value and may not be redeemed for cash.
*Missed appointment fee/penalties are ineligible for the membership discount.
